Our Providers
Jon W. Walker, MD
Dr. Jon W. Walker is a Family Medicine physician certified by the American Board of Medical Specialties. He has worked as a primary care provider since 1989 in Wise County.
Before joining Premier Independent Physicians, Dr. Walker held several positions including Chief of Staff at Bridgeport Hospital, Chief of Staff, Medical Officer, Governing Board Member, and Chief Medical Officer at Medical City Decatur, formerly Wise Health System. He is also the Local Health Authority for Wise County.
Dr. Walker holds a Bachelor of Science in Biology from Texas A&M University and a Medical Doctorate from the University of Texas Medical Branch at Galveston. He completed his residency in Family Practice at the University of Texas Health Science Center at Houston and Montgomery County Medical Education Foundation at Conroe. He is also a Fellow of the American Academy of Family Practice.
When not seeing patients, Dr. Walker enjoys spending time in the great outdoors camping and golfing. He also enjoys working with local school systems and served as the Team Physician for the Bridgeport High School football team from 1991-2019. Dr. Walker has been married since 1992 and has one daughter.

Kaye Puckett, DNP, FNP-BC
Kaye Puckett, DNP, FNP-C is a certified family nurse practitioner with experience providing care to patients of all age groups.
Kaye holds an Associate of Science degree in Respiratory Therapy from Eastern New Mexico University and has earned a Bachelor of Science in Agriculture and Animal Science, Bachelor of Science in Nursing, and Doctorate of Nursing Practice and Family Nurse Practitioner Certification from New Mexico State University.
Kaye began her health care career as a Registered Respiratory Therapist working in Fort Worth, TX, Albuquerque, NM, and Colorado Springs, CO in Neonatal Intensive Care, Pediatric Intensive Care, and the Cardiac Intensive Care Unit. She transitioned to nursing in 2017 and has worked in hemodialysis, Emergency, and Medical/Surgical Units.
Before joining Premier Independent Physicians, she worked as a Family Nurse Practitioner at Wise Health Clinics.
Kaye is available for appointments beginning May 6.
